Eligibility Requirements:

  • Must be a college senior and accepted to medical school or currently enrolled full-time in an accredited medical school program.
  • Must demonstrate personal motivation for excellence in both character and academic acheivement. Personal intergrity, as exemplified by leadership, community involvement and concern for others, will be considered.
  • Must have graduated from a high shcool in San Mateo County or Santa Clara County or have permanently resided in that same area during high school. Must be a United States citizen.

  • Signed and Dated Personal Statement
  • Two letters of Reference
  • Offical Transcripts of Grades
  • Financial Need Documentation
  • Proof of Citizenship or Residency Status
